Introduction: various studies indicate the efficacy of telemedicine projects, but such projects have not become comprehensive yet. The present project was one of the rare qualitative studies to identify the challenges of establishing telemedicine and its integration into health care system.

Purpose: this study was conducted to identify the challenges of establishment of telemedicine in Jahrom University of Medical Sciences.

Methods: conducted through content analysis, the study used semi-structured interviews for data collection. The participants, who were nine authorities from Jahrom University of Medical Sciences, were selected using purposive sampling. Data were analyzed using Granheim and Lundmen Content Analysis.

Results: The main five main themes from the results were technical, organizational, financial, human, and support factors, which were the main challenges for the establishment of telemedicine.

Discussion and Conclusion: Deployment and provision of telemedicine services face many challenges with various factors contributing to the successful deployment of such projects. Currently, we have technical and support weaknesses for the deployment and use of this technology. The provision of a national program and technical infrastructure seems to be the most effective way to cope with the challenges.
